New Life Adult Day Health Care Center and Lily Adult Day Health Care Center have immediate openings for RN and LVN, Social Worker, Activity Coordinator and Receptionist positions.
Adult Day Health Care Center provides an organized day program of therapeutic, social and health activities and services to person 18 years or older, or other adults with functional impairments, either physical or mental, for the purpose of restoring or maintaining optimal capacity for self-care. Provided on a short-term basis, adult day health care serves as a transition from a health facility or home health program to personal independence. Provided on a long-term basis, it serves as an option to institutionalization in long-term care facilities, when 24-hour skilled nursing care is not medically necessary or viewed as desirable by the recipient or his or her family.
Due to the pandemic, the center does not provide congregate services to the participants at this time. Staff are taking a telehealth method and providing very limited direct services to the participants.

REGISTERED NURSE /LICENSED VOCATIONAL NURSE
Responsibilities and Duties
• Conduct initial and continuing assessment of the participant and develop a written individual plan of care.
• Observes and monitors the health status of participants.
• Administer medications or monitor self-administration of medications by participants.
• Supervise personal care given by program aides.
• Maintain participants health record which includes:
(a) signed quarterly progress notes from each member of the multi-disciplinary team and outside consultants.
(b) record of medications and treatments administered.
(c) Quarterly narrative nursing note, unless a participant's condition requires a more frequent note,
indicating participant's progress toward achieving health goals.
• Provide emergency care including arrangements for transportation.
• Notify physician of any marked change in the participant's condition.
• Participate in team meetings and provides consultation to therapists, families and participants.
• Document services provided
• Other duties as assigned.
Qualifications and Skills
• Must possess a current active license to practice as registered nurse (RN)/ licensed vocational nurse (LVN)
in the State of California.
• Preferred experience: One or more years working with elderly population, preferably Korean elderly.
• Must be able to read, write and speak clearly in the English language as well as Korean.
• Proficient in MS Office
• Must be legally authorized to work permanently in the U.S.
SOCIAL WORKER/ SOCIAL WORK ASSISTANT
Position Summary
The Adult Day Health Care Center Social Worker assumes responsibility for assuring the department and all individuals at their direction provide intake and participant advocacy services as well as coordinating social service matters with physicians, staff, participants/patients, families and other organizations. The Social Worker strives to lessen or remove blocks to the participant’s ability to access wellness and the use of medical/rehabilitative care across the continuum of care.
Responsibilities and Duties
• Initial and continuing assessment of the participant by the medical social worker in the formation of an individual
plan of care by the multidisciplinary team.
• Maintain signed progress records in the participant's health record at least quarterly.
• Assumption of responsibility for implementation and coordination of the discharge plan.
• Provide referral to outside resources and assisting with counseling when applicable.
• Provide problem-oriented discussion groups among participants.
• Serve as a liaison with the participant's family and home.
• Provide referral and follow-up for referred persons not appropriate for adult day health care.
• Promote peer group relationship through problem-centered discussion group and task-oriented committees.
• Serve as liaison with other community agencies who may be providing services to a participant and work with
these agencies to coordinate all services delivered to the participant to meet the participant's needs and
avoid duplication. Liaison shall include, but not be limited to the following agencies:
(A) In-Home Supportive Services in the county welfare department.
(B) Home Health Agency providers.

Social Worker Qualifications/ Skills
• MSW (or MA in psychology, gerontology or counseling plus one year of experience) or BSW
(with 2 years of experience in aging, health or long-term care services) from an accredited school.
• Bilingual English / Korean.
• Current Physical & TB Clearance
• Current CPR/First Aid Certification
• Proficient in MS Office
• Ability to work with individuals with physical or mental limitations
• Valid California driver’s license and proof of Auto Insurance
• Must be legally authorized to work permanently in the U.S.
Social Work Assistant Qualifications and Skills
• Bachelor's degree in the social sciences or related fields.
• Bilingual English / Korean

ACTIVITY COORDINATOR
Position Summary
The Activity Coordinator supervises and directs the activity program to meet the needs and interests of each participant. The activity program shall assist the participant to resume self-care and normal activities or to prevent further deterioration.
Responsibilities and Duties
• Participation in initial and continuing assessment of the participant’s needs for the development of the individual0
plan of care by the multidisciplinary team.
• Maintenance of a signed quarterly progress record and individualized activity plan in the participant’s health record.
• Posting of a schedule of planned activities in a central location readily accessible to participant, family, guardian
or authorized representative.
• Involvement of participants in the planning of the program.
• Utilization of volunteers as needed to implement the plan.
• Utilization of adult day health center’s transportation to provide activities in the community as indicated
by participant’s needs and interests.
Qualifications and Skills
The activity coordinator shall have one of the following qualifications:
1) Two years’ experience in a social recreational or educational program within the past five years, one year of
which was full-time in an activities program in a health care, mental health or handicapped program setting.
2) Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist,
occupational therapist assistant or qualified social worker.
3) Completion of at least 36 hours training in a course designed specifically for this position and approved by
the Department and shall receive regular consultation from an occupational therapist, qualified social worker
or recreation therapist.
